Thailand Registration Form

This is a registration form for the Davis Chamber Thailand trip from November 8-16, 2025. We are so excited to have you join us!

Use a separate form for each person traveling. Please verify all information is entered correctly! Davis Chamber & Citslinc International will use this information to book flights, hotels, etc. Accuracy is imperative!

Note: accommodations are based on double-occupancy. If you would like a single room for an additional fee of $800, please type "single" on this question, and select the single room option on the next question.

A non-refundable registration fee of $400 per person is due with your registration and is included in your total tour fare. The remaining balance is due on or before July 31, 2025.

I prefer an invoice be emailed to me to pay online through Square (see options below).
I prefer to mail a check (made out to Davis Chamber of Commerce and mailed to 450 S Simmons Way, Kaysville UT 84037).
I prefer to pay in person at the Orientation Meeting on February 19th (more info below).

Please only include the $400 deposit on my first invoice (due immediately), and then send an invoice for the remaining amount on July 1st (due July 31).
Please send an invoice for the full amount of the trip now.

A valid passport is required. Securing or updating a passport is your responsibility. Passports must be valid 6 months past travel date. U.S. and Canadian residents do not need a visa application. 

A copy of your passport must be sent to the Davis Chamber with your registration form (no later than July 31st, 2025). Please email a copy to mandy@davischamberofcommerce.com.

We encourage you to fly with the group, but if you prefer to book your own flight (whether you plan to extend your trip, upgrade to business class, or ensure you can choose your own seat, etc.), you have the option to do so. Citslinc will deduct $500 off your total tour fare should you choose to purchase your own airfare. You are responsible for any extra transportation, hotel fees, etc., until you meet up with the group.

I would like to book my own flight. Citslinc will NOT be purchasing a flight for me. I recognize I will get a $500 deduction off my total tour fare.

A non-refundable registration fee of $400 per person is required at time of registration. This will be deducted from your total tour fare, the balance of which will be due by July 31, 2025. If you need to cancel the trip for any reason after July 31, you will receive a full refund, minus a cancellation fee of $800 and the $400 non-refundable deposit.

There will be a valuable trip orientation meeting on February 19th, 6 pm, at the Business Resource Center (450 S Simmons Way, Kaysville). This meeting will give detailed information about the upcoming trip and is for any individuals interested in getting more information. Please bring any questions you have to this meeting.
